How to Get the Right Fit: A Sizing Guide for Incontinence Products
If you've ever ordered incontinence products online and ended up with something that didn't fit quite right, you're not alone. Sizing in this category is less intuitive than it looks, and a poor fit doesn't just cause discomfort, it undermines the whole point of wearing a product in the first place.

Getting the fit right makes a real difference. Here's how to do it.
Why fit matters more than you think
An incontinence product that's too loose will leak, regardless of its absorbency rating. One that's too tight will be uncomfortable to wear for any length of time, which means you'll wear it less consistently, and that defeats the purpose.
The right fit means the product stays in place, seals properly, and feels comfortable enough that you forget you're wearing it. That last part matters more than people expect.
Start with your waist measurement, not your clothing size
This is where most people go wrong. Clothing sizes vary significantly by brand, and incontinence product sizing doesn't follow the same scale. A size Large in one brand may fit someone who wears a Medium in another.
The only reliable starting point is your actual waist measurement in inches. Measure around the widest part of your hips and waist (the fuller of the two measurements is what guides sizing for briefs and pull-up underwear). Use a soft measuring tape, or a piece of string that you measure against a ruler.
Write that number down. Then use it, not your pants size, to cross-reference the size chart for whichever product you're looking at.
Pull-up underwear vs. tab-style briefs: fit works differently
For pull-up underwear (the kind you step into like regular underwear), fit is primarily about the waist/hip measurement. The product should feel snug without digging in, and the leg openings should lay flat against your skin without gaps.
For tab-style briefs (with refastenable tabs on the sides), you have more room to adjust. The tabs let you customize the fit across the hips, which is useful if you fall between sizes or have an asymmetrical fit. For briefs, it's generally better to size up if you're between measurements, then adjust the tabs to get the right snugness.
What "snug but not tight" actually means
The product should:
- Lay flat against your body without bunching or pulling
- Have leg cuffs that contact your skin all the way around without leaving red marks
- Feel secure when you move, sit, or bend without shifting out of place
If you're leaving marks on your skin after wearing it for a few hours, it's too tight. If you notice leakage at the legs or waist, it's likely too loose, or the absorbency level isn't right for your needs (that's a separate question from fit).
If you're between sizes, here's what to do
Size up for pull-up underwear. A slightly looser fit is easier to manage than one that pulls or cuts in, and most pull-ups have enough elasticity to hold their position even with a little extra room.
Size down for briefs, then use the tabs to close the gap. The tabs give you more control over the final fit than the base size alone.
If you've tried a size and it's consistently not working, switching brands is often more effective than adjusting within the same line. Different products are cut differently, and some will simply fit your body shape better than others.
A note on overnight products
Overnight products often run slightly larger than their daytime equivalents, even at the same labeled size. If you use a different product at night than during the day, measure and compare size charts separately rather than assuming the same size will apply.
The practical approach
Order a small quantity of a new product before committing to a larger supply. Most people find their best-fit product through a bit of trial, and that's completely normal. Once you find what works, sticking with it consistently tends to be far easier than switching brands frequently.
